The man page for man says that "MANPAGER ... may not use pipes to connect multiple commands". I think that this is an inconvenient restriction. And it seems that man on Mac OS X does not have such a restriction.
This restriction is also inconsistent with POSIX¹, which says:
Any string acceptable as a command_string operand to the sh -c command shall be valid.
