OK, I've found two separate problems here. Firstly, the check that decides whether to apply the major/minor number when activating the device was inverted so it never did anything.
Secondly, if changing the minor number of an existing active device, the update sequence is wrong and the change doesn't take effect till the next time you activate the device.
