[I have some local patches, but they do not affect this.]

I use the imap body caching support.  When my mail server gets sufficiently
unhappy during download of a large message, I sometimes have to kill mutt;
after that point, mutt won't try to refetch the partially downloaded message
unless I manually delete it from the cache.

Probably downloads should go to a temporary filename and be renamed into
place.
