I sent a manpage.xml.ex example [1] earlier. Anyway, I included a very
large example [2][3] to the docbook-xsl package. Maybe you just have a
look at it and adopt the improvements from this example.

One thing, that could be removed from my example: the dhdate entity and
the <date> tags - because the XSL-stylesheets now produce the date
automatically, if no date is provided. I will update this with the next
docbook-xsl package.
