It is apparently possible to add custom headers with the stock
thunderbird/icedove[1]:

To add custom headers to emails in Mozilla or Netscape:
* Open your user.js configuration file in an editor.
* Add 'user_pref("mail.compose.other.header", "{custom headers}");'.
o Replace {custom headers} with a comma-separated list of the custom
headers you want to
be available. "Sender,X-Y" adds the Sender: and X-Y: headers, for example.
* Save the file and close the editor.
Unfortunately, it is not possible to specify any default values or have certain
custom headers
appear in outgoing emails automatically. You still have to enter the values for
the headers manually
when creating messages in Mozilla or Netscape.

This is what I have in my user.js file:
$ cat user.js
# my own settings, EddyP
# add these headers to the list of possible headers
user_pref("mail.compose.other.header", "X-Debbugs-CC,X-Debbugs-No-Ack");
And now the list of headers in the mail looks like the one attached.
Not too great since every new custom header needs to be added by hand
and must restart the app to take effect, but for frequently user ones...
