kqemu default behavior is now default to 'major=0', which assumes UDEV
case, and calls misc_register,as can be seen from
kqemu-linux.c(module_init).

kqemu_dev has MISC_DYNAMIC_MINOR, which means minor number is assigned
dynamically. This means, that device node major/minor number will be
assigned through misc_register randomly when major=0.
Therefore, the device major/minor of /dev/kqemu cannot be determined
before kqemu module is loaded, and thus node needs to be created after
boot sometime in the init scripts.
One proper way to solve this would be to get lanana to register kqemu
module, for the time being, kqemu is unusable for non-udev systems.
