sorry for not answering earlier.  Currently we're being drowned in
release-critical and important bugs, and this one just got too low a
priority.

This happens with teTeX-3.0, too, and although I cannot test it at
present, I bet it also happens with TeX Live 2005.  

The AMS stuff is getting bugfixes, but only slowly, index is not
maintained any more IIRC.  Therefore I fear there's not much hope that
it will be possible to use both together soon.  Have you tried
makeidx.sty instead?
