Whether or not the addresses are case sensitive or not is dependent on the receiving MTA; whether the BTS chooses to do seaches which are case sensitive or not is entirely its choice. What it must do is preserve the case of the email address when it is *sending* mail or similar. So in short, #1 is not technically incorrect and is the method that will be utilized.
