It can happen at any level of volume. For instance, I might have the volume at 76%, turn it up a notch to 80%, and have it show as muted instead. I have not observed any correlation with volume. As I change the volume up and down, I can see the same levels of volume appearing sometimes as muted and sometimes as the correct level. - Josh Triplett
