The problem is not that the Debian patch disables the default, but instead that alpine checks for the existence of /usr/sbin/sendmail at build-time and uses that if it finds it, and if it doesn't find it at build time

That is, unless you add an argument to ./configure with the right path, which the next Debian version bump will do.
