My scanner has a sheet feeder, but not duplex, so I've got no way of reproducing this. But - you are the first person whom I know has tried a duplex scanner with gscan2pdf. The single-sided/double-sided settings are designed to sort out the page numbering for non-duplex sheet fed scanners scanning all the front pages, then all the reverse pages. If I was to use a duplex scanner, I would set gscan2pdf to single-sided, but the source to duplex. Have you tried this combination?
