I had the same problem, and made apt-cacher work when I renamed
/var/cache/apt-cacher/sums.db out of the way.
I tried your patch in order to give you more informations, find my
error.log attached.
In fact, I noticed a problem on the cron-daily report, and I have it
again now, maybe it could be linked :

I tried again apt-cacher-cleanup.pl -r but I have the same error,
(that's why I renamed sums.db a few days ago) :

Could not read config file: No such file or directory at
/usr/share/apt-cacher//apt-cacher-lib.pl line 39.
This time, I (quick and dirty) patched apt-cacher-lib.pl in order to
make it work again :
