Would it be possible for you to run it under debugging all the time?  I know 
this could generate a huge amount of data.

It may be useful to do something like:

offlineimap -d imap -1 -u TTY.TTYUI | gzip -9 > logfile

or -u Noninteractive.Basic may be useful

or you could use tee with a named pipe that runs through gzip...

What do you think?

I don't think it will be possible to track this down without this log.
