According to <http://www.mail-archive.com/[EMAIL PROTECTED]/msg00033.html>, this may also be caused by the use of SuExec:
*Hint 2 processmanager signal problem:
The processmanager changes the euid only when the suexec wrapper
is enabled. The module cannot send the TERM signal to the php
process when the interpreter uid has been changed..
