Unfortunately, I can't reproduce this here. See the attached results
that ... well ... don't look really nice but reasonable. The first is
the result for your plain (incomplete latex) input file, the second is
from the version rounded up with an appropriate latex header/footer.

Please provide additional information about your setup:
e.g. /etc/latex2html.conf, etc.
