We finally tracked this down:

I once upgraded the box to testing by mistake and then downgraded back to
stable/etch. Somehow (don't ask me why), the rules files stayed at the
newer version.

One of the differences is the support for the ACTION "change" and the "|"
operator, as used by the statement:

ACTION!="add|change", GOTO="permissions_end"
[... permission settings ...]
LABEL="permissions_end"

Using such scripts with the stable udev *always* executes the GOTO, thus
ignoring all permission settings.

So: *if* there really is a bug in udev, it's in the packaging. I'm not sure
whether downgrading is officially supported or not.
