I expected
\indexentry{keyword|mac}{1}
in .idx to be converted to
\item ... keyword ... \mac{1}
in .ind, but we get
keyword ... 1}
which is lacking "\mac{".
I am not quite sure if this is a bug, but I'd appreciate if you
investigate this issue sometime.
