Using the PulseAudio output plugin (in plugins-extra, with pulseaudio 0.9.15-4.1), every time the playlist advances, volume decreases. Using the PulseAudio volume control I just saw 64% -> 31% -> 0, 80% -> 48% -> 15% -> 0%, 100% -> 67% -> 35% -> 2%: call it a decrease of about 32 volume percentage points each new track).
