My PulseAudio mixer frequently comes up muted and at 50% volume. And the panel 
mixer icon shows this.
But when I left click on the icon, unmute it, change the volume, close the 
mixer app, the panel icon remains muted.

But if I right click, choose settings, close that: then the icon gets back in 
sync with the mixer.

The same thing happens in reverse if I mute the mixer.
