I accidentially had a repeated:

session optional        pam_mount.so

line in common-session, causing pam_mount to segfault and all logins to
fail.  I didn't get a backtrace, but it should be trivial to reproduce.
(Yes, it's user error, but pam_mount shouldn't segfault even then.)
