I had put this problem on the back burner, and recently looked into it again. Turns out that somehow an extra blank line had been inserted into the top of managesieve.lib.php. I guess this happened somehow when I was moving the web site from the old server. No idea how, but making <?php be the first line in the file, instead of a blank line, made everything work just fine again.

managesieve.lib.php contains only comments in line 2 unless you changed
it. There shouldn't be any output at all. I'm sorry, I can't see how to
