During the first configuration of ekiga (wizard) I selected my device as
ringing and sound out device, i.e. ATI IXP (PTLIB/ALSA). My initial tests
showed ekiga worked well. However I noticed that when mixing sound from
different sources (totem, iceweasel, mpg123, etc) and ekiga (I tried this
with the echo test) I got an error that sound device for output was
unavailable. On the other hand, if I only run the echo test in ekiga, but
then try to play a sound with another program, the latter is muted.

I decided to select (under configuration) as ringing and audio out, the
option: Default (PTLIB/ALSA). By doing this, sound mixing is working.
Note: I have installed the base default system which does not install all
pulseaudio (only libpulse0).

In the new release the Default device will be selected by default, http://git.gnome.org/browse/ekiga/commit/?h=gnome-2-26&id=f1d73cbb. I think solves this issue.

I suppose that ATI IXP device opens it "exclusively"or something like that (I do not know well how sound works), hence your problem.
