On 2.6.22 kernel the USB device goes away (is deconfigured) when the system is suspended. When your root is mounted from the device you get those errors about IO to dead device. When you resume (or if USB is reconfigured during suspend then even during suspend) the sda device is dead but still used so the USB drive is configured as sdb. Nothing hibernate can do about this. This should be working better with recent kernels and non-broken USB controllers.
