In practice, one can click "Cancel" (or authenticate) in the visible authentication dialog. Then Iceweasel appears to be frozen. But if one lowers the browser window or looks at the window list, one sees a second authentication dialog. This bug is always reproducible, even in safe mode (-safe-mode option). Upstream Firefox 4 beta[*] doesn't have this problem (only one authentication dialog appears, above the browser window).
