I was getting yelled at by my Exchange administrator for having too many
open connections and almost single handedly bringing our Exchange server
down.  Despite setting maxconnections = 1 and turning off keepalive.
I've had to stop using autorefresh and just call single passes
offlineimap in a from a shell script, so that IMAP connections are
actually closed.  That has mitigated the problem for now.  Now I'm sure
this is Exchange's fault, but it still seems like something offlineimap
should be able to handle. 

We're running Exchange 2007.
