I have the same problem with fluxbox version 0.9.12-1 (in unstable).
update-menus as root or user and retstarting fluxbox does not help.
the menu is updated in wmaker. removing ~/.fluxbox and restarting fluxbox helps,
so probably some auto-generated per-user config file unintentionally overrides
the debian system menu.
