I can confirm this has affected me too after upgrading from mysql-server
to mysql-server-4.1 however, my syslog output is somewhat different.
here's a snippet:

In answer to your other questions:
/var had 1.2G free

old_passwords = 1 was set, changing it made no difference for me.
