ok, the only thing i can recommend then is to see if you can get some
debugging information if it happens again -- strace output, packet
capture, "unbound-control dump_infra" output, increase the logging
verbosity ("unbound-control verbosity"), etc.

do you recall any other specific versions that this happened on?  there
was a rare new upstream version update in lenny (from 1.0.2 to 1.4.6
iirc), do you happen to remember if you saw it before 1.4.x?
