Neil, could it be that 

$MDADM --assemble --scan --run --auto=yes

cannot assemble arrays whose ARRAY definitions have metadata= or
name= in the mdadm.conf file? I am not in a position to test this at
the moment, unfortunately...

You can find the corrected initrd at

If you replace the (manually fixed) mdadm.conf in it with the
automatically generated

it exhibits the problem.

IIRC (I had a good grasp at the time, but that was two weeks ago:-/)
the problem is not that the script is wrong but that the

$MDADM --assemble --scan --run --auto=yes

step does accept ARRAY lines with "metadata=1" and/or name='c8':1
