The README actually says that it was written for PHP 4, but uses the .php3 extension for historical reasons. This is a case of hysterical raisins :) I caught this because I didn't think of adding that ancient extension to the Apache setting 'AddHandler php5-script .php .inc'. Then I remembered to switch to matching the application/x-httpd-php content type, but that also doesn't catch *.php3 - I'd need application/x-httpd-php3.
