Today I solved the issue by upgrading my client to mount 2.12r-15, and
removed the NFS user server on the server and replaced it with the NFS
kernel server.  After performing some changes as shown at
http://wiki.debian.org/SecuringNFS and tweaking the Shorewall firewall
rules script, NFS is working well.

This bug can be considered solved until further notice.
