The only difference between apache 2.2.3-2 and 2.2.3-3.3 that appears relevant is that 2.2.3-3.1 *enables* authz_user by default, which is supposed to fix the first part of 394097. Following the other instructions on that page, to use AuthBasicAuthoritative off, works perfectly well for me.
