Concerning the /var/tmp (and /tmp) cleaning, the Debian specific patch is https://salsa.debian.org/systemd-team/systemd/-/blob/debian/master/debian/patches/debian/Bring-tmpfiles.d-tmp.conf-in-line-with-Debian-defaul.patch?ref_type=heads

This patch/change was added in 2012 and has required very little to no maintenance since that time. So the cost of keeping it is very low maintenance wise.
