I've been unsuccessful in trying to reproduce this in a kvm virtual machine so far. I understand it from IRC that you had a successful VirtualBox installation using the same configuration.
This make me suspect there is some hardware related race condition. Could the problem be that the /dev/cdrom device show up too late with your hardware?
