Thanks again to everyone involved. I tried swapping the SCSI card from the problem machine with another Itanium, and the SCSI POST screen came up fine (which never happened before) but it doesn't come up on the other Itanium now. Before the swap, however, I was able to get the cd image to run a successful install on the other Itanium. In short, the problem does, in fact, appear to be with the SCSI card, and not at all with the installer or ia64 port.
