I currently use a portproxy (netsh interface portproxy) on XP
to tunnel my Outlook. This works with Courier IMAP-SSL and
Postfix + TLS, the smtp headers still reflect the correct host
as the IPv4 -> IPv6 "conversion" happens on the same host which
would otherwise do the IPv4 connect to the server.

Same trick can be done with other products and on unix boxes
ofcourse ;)
