You need to add a tag to /etc/tomcat4/server.xml inside <Host>:
<!-- Tomcat Manager Context -->
<Context path="/manager" docBase="manager"
debug="0" privileged="true"/>
I'll add this to the default configuration for tomcat4 4.0.3-1.
