Have you got kwalletmanager ? It seems to use a different set of controls from normal form completion, and (though I've not exhaustively experimented) I've not met anyone who can say where its options are. However you can change or delete the kwallet entries individually if you open the wallet, go to "Form Completions", and find the URL in question.
