Dear Maintainer,

after upgrading to any kernel above 3.x, such as 4.9 or 4.16
on booting, qla2xxx module fails with:

Failed to initialize adapter
Setup chip ****FAILED****
ISP Firmware failed checksum

this happens with qla2100 and qla2200 FC cards. This happens regardless of the 
FC cards BIOS settings about loading RISC code.
This happens regardless of which version of firmware-qlogic is installed, from 
what I have found.

After going back and booting a 3.x kernel such as 3.16 , the problem is not 
there.

without resolution.
