I just tried
update-initramfs -k 2.6.18 -u
when I didn't have an initrd, and got the error:
/boot/initrd.img-2.6.18 has been altered. Cannot update.
It should instead tell me that the file was missing. Changing -u to -c
made it create the initrd as normal.

got fixed inbetween, saying it can't update that initramfs as not
existant. marking as closed thus.
