Andrew Vasquez on linux-scsi pointed out http://git.kernel.org/?p=linux/kernel/git/torvalds/linux-2.6.git;a=commitdiff;h=031e134e5f95233d80fb1b62fdaf5e1be587597c The patch applies without any change to 2.6.26.6, and indeed fixes the problem. I don't know (yet) why it wasn't incorporated into 2.6.26.6.
