Hi Jonathan, thanks for your reply! Fortunately, the problem is no longer present, it turned out that it was caused by a hardware failure. After migrating all data to software RAID and removing the HW RAID controller from the system, the issue never happened again (all other hw components remained the same), I think it's safe to close the report.
