I hit this yesterday too, on kernel 3.14-2-amd64 (3.14.15-2). With RAID1 + LVM + ext4, I added a USB disk to the RAID1 and started to get immediate errors while mysql wrote to disk:

It seems that this bug was tracked at: https://bugzilla.kernel.org/show_bug.cgi?id=9401 which was marked as "fixed" back in 2013. But the comments suggest that Kent Overstreet's immutable biovec patchset was the solution, which appears to have been already included in 3.14.
