I was under the impression that "-k all" meant "all *installed* kernel versions". update-initramfs(8) speaks of "all kernel versions, that are already known to update-initramfs". May you have run into a bug in update-initramfs? Besides, I can not reproduce the issue you're experiencing; I'm building a Squeeze system using live-build 2.x and initramfs-tools from sid (0.99).
