Dictionaries as used in *spell are compiled from word lists. OpenMedSpell is one. SCOWL provides a bunch of others. They need to be combined and compiled to be used as *primary* dictionaries for spellers. Most spellers will allow for using one or more user-local "simple word lists" in addition to the installed dicts.
