No idea. Later versions of Apache let you specify a configuration directory where you can deposit any configuration file and it will be read during start up. I've got a variety of versions of Debian here, and the most recent support /etc/apache/conf.d/ which would be a very suitable place to drop the 'modcaml.conf' configuration file.
