If you don't use fam (because you don't want another process and some
overhead), class KDirWatch pool each 5 seconds many directories. I use
kernel 2.6-testX, and a process kdeinit takes several minutes of process
time. See my line of top, it's just after 10 minutes.

Could you enable dnotify ? it's just to add --enable-dnotify when you
call ./configure ...

I know i can increase the pool interval, but a prefer a native linux
solution.
