One of my packages has been renamed completely (the project name

You can rename a repo in settings -> general -> advanced and it should be available on the old address as well (afaik). So I would go for the rename.

2. What should I do with the changelog? Leave the current entries as
they are now or start one from scratch.

Leave them in, same as the git history.
