Could you send me the docbook file? Currently, I can only reproduce such a bug (because it's clearly look the same) on a i386 with xmltex (and not pdfxmltex). Again, I'm pretty sure this is related with the \special command which confused tetex, but I will even more appreciate it if I can reproduce the bug on both xmltex and pdfxmltex, especially on my PowerPC.
