In regards to issue #2: I created an identical VM and performed an identical install, except for one difference, I used ext4 instead of btrfs (for all of /, /boot and /home). This install works fine.
I noticed that in the broken install, initramfs is missing cryptsetup. I'm not sure if there is anything else missing, I may compare further later. I recreated the broken install from scratch, and it still exhibits the same issues, so that disproves the possibility of a package update having fixed things. So d-i does not seem to be adding cryptsetup to the initrd when btrfs is used (at least in this scenario).
